USA registration forms:

You will need a tryout waiver and release form signed and filled out for tryouts.  (2nd link)

All junior members of Rally Force can now fill out their USA memberships forms online by clicking on the 3rd link, and print and bring to the first day of practice.

All coaches and adult members of Rally Force can also fill out all forms by clicking on the 4th link.

All Short Sets members should fill out the forms in the 5th link.

Rally Force Club Members

All club members can access the USA forms online directly from the USA Great Lakes region web site.

Please fill out all forms (link 3) for juniors and (link 4) for adult members and bring to the first day of practice.
